Through a collaborative effort involving Sangamon County physicians and safety net agencies CATCH seeks to enroll low-income, medically uninsured residents to have access to coordinated,medical services including access to physicians,medical testing,screening and prescriptions medications. Each person enrolled in CATCH will have a primary care medical home and timely access to other medical services they need. As a result, the physical and mental health of the population enrolled in CATCH will improve, and the resources of the community will be used with optimal efficiency.
